All copays are due at time of appointment;
All self-pay accounts are payable at the time of service;
We accept assignment of insurance benefits for all insurances. The balance is your responsibility. You will receive a statement regarding your balance after insurance has paid. This amount is due upon receipt of the statement.
We accept cash, check, Visa and Mastercard.
